About this ebook

New Psychoactive Substances (NPS), also known as “bath salts”, “legal highs” and “research chemicals”, are defined as drugs that that potentially pose a public health threat but are not yet listed and controlled by the United Nations drug conventions. NPS can be divided into several subgroups of which synthetic cannabinoids and synthetic opioids, where the later including fentanyl analogues, are two common groups. These NPS are often designed to mimic the effects of established drugs such as Δ9-THC or morphine/fentanyl. The emergence of these substances on the illicit drug market occurs mainly as they can serve as legal alternatives to mature, already classified narcotics. Because of the significant number of negative effects associated with synthetic cannabinoids and the high potency of fentanyl analogues, both types of compounds have been linked to numerous intoxications, both fatal and non-fatal. Forensic toxicologists and authorities have a hard time keeping up with the fast emergence of NPS, as there is always a new compound that replaces a recently classified one. To combat the serious threat to human health linked to NPS, various measures can be undertaken. To establish NPS identity in toxicological cases, the availability of reference substances is necessary. Evidently, reference substances are often not available as the chemical structures of NPS are initially unknown.

Urine is by far the most common matrix for drug detection, as the sampling is non-invasive and its window for detection of drugs is wider than for blood. However, the parent compound is often either absent or present at low concentrations in the urine.  In the case of synthetic cannabinoids, typically only a small amount of the parent drug is excreted via the urine meaning that using the parent as a biomarker for intake is insufficient. The use of formed metabolites as biomarkers is therefore a better option. An understanding of synthetic cannabinoid metabolism is therefore critical. In the case of fentanyl analogues, metabolites will make the identification in urine more straightforward, but parent compound might still be present at higher concentration that makes the need for metabolite less critical. However, it is still valuable to understand their metabolism as the metabolites might be active µ-opioid receptor agonists. Given the importance of understanding the metabolism of NPS several studies have been carried out. Many of these studies have been carried out by applying in vitro techniques, such as human liver microsomes or human hepatocytes, while some studies have had access to urine samples. For the samples from the in vitro or in vivo studies, a chromatographic technique coupled with a mass spectrometer has commonly been utilized to separate components of the metabolic mixture in the samples, and to elucidate their structure. However, their structures can in many cases not be fully disclosed given that the mass spectra of certain positional isomers will be indistinguishable.

The aim of this thesis was to identify the chemical structures of key metabolites of selected synthetic cannabinoids and fentanyl analogues. This was achieved by conducting metabolism studies using human hepatocytes and liver microsomes to tentatively identify the formed metabolites, followed by synthesis of reference substances to enable confirmation of the exact chemical structure of the metabolites. Once identified, the key metabolites can be produced in a larger scale and be made available as reference substances for the forensic community for use in routine urine analysis, as well as in toxicological studies.

Numerous potential metabolites across different synthetic cannabinoids and fentanyl analogues, such as ADB-HEXINACA, ADB-4en-P-5Br-INACA, ADB-P-5Br-INACA, acetylfentanyl, acrylfentanyl, cyclopropylfentanyl, isobutyrylfentanyl,  4F-isobutyrylfentanyl and 3-phenylpropanoylfentanyl,  were synthesized and characterized. Upon comparison with results from in vitro studies using human hepatocytes, with or without human urine samples, the structure of key metabolites could be revealed. Synthetic methods for scale-up synthesis of these key metabolites were also developed. Moreover, the knowledge collected in this work may be extrapolated to new emerging substances to predict their metabolism and the formed metabolites.

NPS kan delas in i flera undergrupper.
